Pick from four Physicians Formula products: Eye Booster Lash Boosting Felt-Tip Eyeliner Marker + Serum ($10.95); Youthful Wear Cosmeceutical Youth-Boosting Foundation ($14.95); Youthful Wear Cosmeceutical Youth-Boosting Concealer ($12.95) or Organic Wear Lash Boosting Mascara ($9.95).

Purchase your choise of product labeled with the "Try Me Free," sticker, which will need to be mailed with the completed rebate form and receipt by the dates specified.
